We're on a mission to improve the lives of people receiving care and their caregivers everywhere. Your help with our testing will make a giant difference in us achieving this goal. We're so excited to have you here! Thank you for your interest in testing the Threshold Care experience.

Inside this Project, you'll find a survey to fill out with information that allows us to select the right testers for our projects. You don't have to fill out all of the questions, but skipping some of the questions may affect your likelihood for selection for a test depending on what we're looking for. If you’re selected, you’ll receive an email from us. Please be sure to allow emails from beta@threshold.care to ensure their proper delivery!

Regarding your application:

• Information provided by you will only be used for tester selection and to better understand your experiences with our products. 
• We will not share your demographic information beyond strictly necessary Threshold Care employees.
• If you choose to opt-out of this community or you request to be removed, all demographic information will be removed from the system automatically.
• Please ensure that all information you provide is current and accurate. Take special care if you’re applying to test anything that would require shipping or delivery (such as hardware devices).
• If you are not selected for your first test, don’t worry! We have many more opportunities coming in the future. If we need additional information for future tests, we’ll post a new application survey and inform you via email.
